Student-Athlete Forms

Western Technical College follows the NJCAA compliance/eligibility protocols to ensure a safe and fair playing environment. The following documents are required for all student-athletes prior to being eligible to compete at Western. 

Preparticipation Physical Evaluation 

All student-athletes must have a physical form completed by a medical provider and submitted to the Athletic Department prior to participation in any athletic participation. Forms must be completed annually, performed, and signed by a Doctor (MD or DO), Physician Assistant, or Nurse Practitioner. 

NJCAA Physical Examination Form

NJCAA Eligibility Portal Registration

The NJCAA Eligibility Portal Registration is a required first step to be eligible to compete in athletics. The document linked below has a QR code that can be used to complete the registration process with the NJCAA. This process is used to upload and submit official eligibility status with the NJCAA. This process must be completed and submitted to the Athletic Department prior to the start of practice or competition.

The NJCAA Eligibility Affidavit is a required document for the academic eligibility process. This document must be completed and submitted to the Athletic Department prior to the start of practice or competition.

The NJCAA Amateur Questionnaire is a required document for the academic eligibility process. This document must be completed and submitted to the Athletic Department prior to the start of practice or competition.

MCAC Student-Athlete Code of Conduct

The MCAC student-athlete code of conduct is required to be completed by all student-athletes prior to competition in any MCAC conference event. The conference code of conduct lays out the expectation for all coaches and players competing in the conference.
