Preparticipation Physical Evaluation
Student-athletes competing in Volleyball and Baseball must have a physical form completed by a medical provider and submitted to the Athletic Department prior to participation in any athletic participation. Forms must be completed annually, performed, and signed by a Doctor (MD or DO), Physician Assistant, or Nurse Practitioner.

NJCAA Eligibility Portal Registration
The NJCAA Eligibility Portal Registration is a required first step to be eligible to compete in athletics (Baseball/Volleyball). The document linked below has a QR code that can be used to complete the registration process with the NJCAA. This process is used to upload and submit official eligibility status with the NJCAA. This process must be completed and submitted to the Athletic Department prior to the start of practice or competition.
